DentaQuest describes itself as the largest Medicaid/CHIP dental benefits administrator in the US and has been a Sun Life U.S. company since June 2022. Its legal and operational role varies by state and product: it may be a dental plan, an administrator, a subcontractor, or the operator of selected network and claims functions. Do not infer the required enrollment, contract, credentialing, affiliation, or claim route from the DentaQuest name alone. Payer portals, forms, IDs, and procedures change frequently. Timely-filing limits and other participation terms come from the applicable current payer materials and your executed agreement. Use this page as a starting point, then verify the product, entity, provider, location, and effective date with the payer.

Who they are

DentaQuest’s role varies by state. In some programs it is an at-risk prepaid dental plan (Florida, Texas); in others it processes claims while the state pays them (South Carolina).3 The label on the member’s card matters less than which entity adjudicates and which fee schedule applies.

Networks

DentaQuest operates separate provider networks for state Medicaid and CHIP programs and for its Medicare Advantage, marketplace, and commercial products. A group does not join one national DentaQuest network. Participation depends on the specific program and product.

Determine the participation stack

There is no universal two-application DentaQuest path. Build the stack from the state program, product, service area, member population, and DentaQuest’s assigned function:3
  1. State Medicaid enrollment or screening. Determine which billing entities, rendering dentists, specialties, and locations the state requires to enroll or affiliate, and under which identifiers.
  2. Plan or administrator participation. Determine whether the contract is with DentaQuest, an upstream medical managed care organization (MCO), another dental plan, or the state, and whether DentaQuest performs contracting, credentialing, roster maintenance, or only administrative functions.
  3. Product and affiliation. Confirm the specific Medicaid, CHIP, Medicare Advantage, marketplace, or commercial product; link each treating provider and location to the correct billing entity and network.
  4. Effective dates. Obtain written effective dates for the contract, credentialing decision, provider/location affiliation, and electronic transaction routes. Approval in one field does not establish the others.
Start with the state Medicaid dental page, member product materials, and the current DentaQuest or upstream-plan provider manual. A national checklist cannot capture state procurements, product differences, or regional arrangements. During an acquisition, ask the state, plan, and administrator for written change-of-ownership instructions. Billing notes

  • Claim routing is program- and transaction-specific. The applicable manual may direct an 837D to a DentaQuest payer ID through a clearinghouse, a DentaQuest portal workflow, an upstream MCO, or a state fiscal agent/MMIS. Confirm the payer ID, billing and rendering identifiers, product, location, and effective date before sending a claim.3
  • Prior authorization applies only where the governing program or product requires it for the service and facts. It is distinct from a voluntary commercial predetermination. See Get predeterminations.
