# Liberty Dental Plan

> Liberty Dental Plan profile: Medicaid, Medicare Advantage, and commercial dental administrator, and one of Florida's two statewide SMMC dental plans.

**Liberty Dental Plan** is an independent dental insurer and administrator for **Medicaid, Medicare Advantage, and commercial** products. In Florida, it participates as one of the statewide dental plans in the Statewide Medicaid Managed Care (SMMC) dental program.

Payer portals, forms, IDs, and procedures change frequently. **Timely-filing limits and other participation terms come from the applicable current payer materials and your executed agreement.** Use this page as a starting point, then verify the product, entity, provider, location, and effective date with the payer.

For Florida Medicaid, the operative document is Liberty's **[Florida Medicaid Provider Reference Guide](https://www.libertydentalplan.com/Resources/Documents/ma_FL_Medicaid_Provider_Reference_Guide.pdf)**; enrollment, preauthorization, claims, and appeals rules for the SMMC dental program live there, not in a generic national manual.<sup>2</sup>
